AISI’S MEMBER REQUIREMENT APPLICATION FORM


I. Application Letter consists of :

1.1. Original application letter has to be signed by the Company’s President and

       wrote on the related company’s paper with letter head.

1.2. Original application list must be completely filled and signed by the Company’s

       President on Rp. 6.000,- duty stamp.

1.3. List of Directorate, Commissioner Board, and Experts;

1.3.1. For Directorate Member and Commissioner Board,  enclosed with a copy of

          identification card and curriculum vitae, signed on a Rp. 6.000,- duty stamp

          by the related person.     

          
1.3.2. For experts, minimum for 5 (five) person, enclosed :
1.3.2.1. Acknowledgement letter from related expert, which at that time is

             employed on your company since the date… … and is not employed

             by another company. This acknowledgment letter is signed on a Rp.

             6.000,- duty stamp and acknowledged by the related Company’s

             President. 

             1.3.2.2. Expert’s curriculum vitae that signed on a Rp. 6.000,- duty stamp by

             the related person and acknowledged by the Company’s President.

1.3.2.3. Copies of school certificate and other certificates that owned by the

              related expert.                  

1.3.2.4. Letter of necessitate passing (Surat lolos butuh), for expert that used

             to work for other surveyor company.

1.4. References form 2 (two) AISI’s member companies.

1.5. Acknowledgement to willing to obey the AISI’s statutes and post-secondary law

1.6. Domicile letter ;
1.7. Photos of Company’s President and other Directorate at the size of 4 x 6, two plies

        of photos for each person.


II. Besides the application letter, also enclosed :

2.1.  A copy of Surat Izin Usaha Jasa Survey (SIUJS) from Direktur Bina Usaha and

        Company’s Registration (Pendaftaran Perusahaan), PDN, Ministry of Commercial

        (Departemen Perdagangan).
2.2.  A copy of Surat Izin Usaha Perdagangan (SIUP) from Regional Office of Ministry

        of Industrial and Commercial Affair (Kanwil Departemen Perindustrian &

        Perdagangan).
2.3.  A copy of Company’s Founding Certificate (akta pendirian perusahaan) and

        also the changes.
2.4.  Organizational sheme.
2.5.  A copy of NPWP.
